ELDER CARE DIRECTIONS ARRANGES VARIETY OF SERVICES

After conducting a careful assessment of an individual’s need for assistance, Elder Care Directions can help arrange a variety of services.  Elder Care Directions will then act as a liaison between the client and those contracted to provide services.  The goal is to preserve independence and maintain the safety and dignity of the client.  

Elder Care Directions can provide or arrange for the following services:

Counseling services – Elder Care Directions provides support 
counseling for both clients and their families

·        Home nursing services – home healthcare professionals can be contracted to provide a variety of in-home care services

·        Meal planning and preparation – outside agencies can be contracted to provide delivery of groceries or fully prepared hot meals 

·        Shopping – assistance can be provided for grocery and personal shopping

·        Transportation needs – available for medical appointments, social and community activities

·        Escort to medical appointments – accompany client to doctor visits and serve as a liaison between the client and medical professionals

·        Medication – Elder Care Directions can arrange for both the delivery of prescription medication along with monitoring of a client’s medication schedule

·        Social recreational activity planning – enjoy companionship and social time at local senior centers and other facilities

·        Home maintenance – home repairs, housekeeping, lawn care and pool service

·        Bill paying/household management – assistance with insurance claims, monthly bills and correspondence

·        Medical equipment – rental or purchase of necessary home healthcare supplies and equipment

·        Assistance with daily living skills – coordinates with licensed home healthcare agencies to provide certified nursing assistants who can help individuals with dressing, bathing and other necessary activities

·        24-hour emergency coverage – an Elder Care Directions representative is on-call 24-hours a day and is available to follow up on emergency healthcare issues

·        Tax and legal assistance – Elder Care Directions works with a network of professionals who provide these services and are familiar with issues facing seniors

·        Alternative living arrangements and moving assistance – Elder Care Directions makes recommendations and arranges for placement at retirement communities, assisted living facilities and nursing homes

·        Nursing home and assisted living facility monitoring – For clients in nursing homes or assisted living facilities, Elder Care Directions provides follow up consultations to ensure these facilities provide the appropriate services

·        Home safety evaluation – determine what physical changes are necessary in a home to ensure a safe environment
