Elder Care Directions offers geriatric care management to seniors living in the Fort Lauderdale, West Palm Beach and Miami areas of Florida. Geriatric care management is a service designed to evaluate the special needs an individual may require in order to maintain their independence and plan for their long-term care needs. The numerous services available to assist in this goal can be arranged by the care managers of Elder Care Directions.

Elder Care Directions acts as an intermediary between client and the care and service providers to ensure that assistance is delivered in an appropriate and timely manner.   Elder Care’s managers are professionals in the healthcare field and are trained and experienced in providing assessment, care management, home assistance, nursing home and assisted living facility monitoring, counseling, crisis intervention and healthcare education. 

These services are provided on a contractual basis following a self-referral or a referral from a concerned family member, attorney, hospital, physician, or other social agencies.  This site further explains geriatric care management and how Elder Care Directions may assist your family.
